What forms do I need?
In order to get personalized Blues license plates, you need to submit the following forms to the Missouri Department of Revenue:
- St. Louis Blues Emblem Use Authorization Statement: Provides documentation that you have already paid the annual fee to the Blues.
- Missouri Personalized and Special License Plates (Form 1716): Gives you the opportunity to specify what you would like on your personalized license plates.
- Both forms are available by calling 314.589.5884.
How many characters can I put on my personalized plate?
Personalized plates can include up to (6) characters in length. If a license plate is requested which includes a space, hyphen (-), or apostrophe ('), the letters and/or numbers shall then be restricted to no more than five (5) characters, excluding the space, hyphen or apostrophe. I registered my car. Will I have to register it again?
No. The St. Louis Blues plates will simply replace your current plates. Registration stickers will come with your new plates. You will need to turn in your current plates upon receiving your new Blues plates.
I already have personalized plates. Will I lose my plate?
No. Your personalized plate is yours to keep unless you fail to renew it each year. Your current personalized plate will simply be placed on a Blues plate providing it stays within the parameters of the six-digit limit.

How long is my emblem authorization form good for?
The Emblem Use Authorization Form is valid for one year and can be used from the time it is purchased until the time when the plates expire. (Not to exceed one year from the date the letter was issued).
Will the Blues contact me when my Blues plates are about to expire?
No. The Missouri Department of Revenue will send license renewal notices when plates are about to expire. Please allow enough time to receive your Emblem Use Authorization Form. The end of the month is busier than the beginning, so please plan ahead.
